I've been wondering for a while whether it would really be that hard to
have mutt itself deal with generation of flowed text somehow (at sending
time)....

Obviously Pine has some advantages since they are counting on the
builtin editor, but from a /quick/ test, it seems that it generates
flowed text properly using vim as the editor as well.

(Side note - I really like Gary Johnson's stuff_all_quoted patch for
displaying format=flowed stuff correctly, and (optionally) for quoting
it "normally" when not generating flowed text.)
