Works for me(TM)

And this is in my config:

| set header_cache=/home/sanders/.mutt/header_cache/

Does the OP use one monolithic configuration file? 
Is he sure this value is set correctly?
Typing ':set ?header_cache' should give results.
Are the permissions on the directory okay?
Can his user create files and/or directories there?
