I think this should (ideally) be configurable either way. Personally,
this "problem" is one reason I like Maildir. If I haven't opened a
message, just the folder it's in, I don't consider it "seen" or "old"
yet - it's still new to me. I like the fact that I can say "ignore this"
but still have mutt realize that there's new (i.e., unread) mail, even
if I have opened the mailbox since new mail last came in.
 
Presumably mutt could approximate this with mbox by touching a folder
when leaving it if any new messages exist - maybe that's an overly
simplistic approach or there's a flaw in my logic...

That can work to your advantage as well - just depends how you have
things setup.

I use the poorly named "unordered" to put my mailboxes in order of
priority, then cycle through them with a single key macro. If I want to
ignore a given mailbox, I can always tag all the new messages and make
them old or not "new".
