Not with shift-arrow stuff. You use "tagging" (t by default). The cool 
thing about this is that you can tag by pattern if you want to as well 
(e.g. tag all messages containing the word "foo").

Er? You mean, you want to save the attachments into your home 
directory so you can view them? Get into the attachments menu (v by 
default) and save them from there (s by default).

The setting you want is:

    set menu_scroll

That's a pretty old setting (I see mailing list references to it back 
in 2000), but I don't know exactly when it got added to the mutt devel 
tree. Try it, if it doesn't work, upgrade.

With difficulty. You'd need to set up some combination of mutt macros 
and delivery filters.
