It's not well documented (at all) yet.

Basically, a group lets you define an arbitrarily named set of 
addresses, which can then be used in patterns, such as '%C buddies' 
(which would translate to 'messages either to or cc'd to an address 
that matches one of the patterns assigned to the "buddies" group').

Search the mutt developers archive - there's a thread on there 
starting with a message from Thomas Roessler, dated January 9th, 2006, 
that explains it.
