Newer versions of mutt (with ncurses) do this by default ("mono
indicator reverse").
patch-1.3.23.mgs.context_colored_indicator.1 by Mike Schiraldi
will apparently do it for older mutts.
Or maybe 'set arrow_cursor' is an option for you.
