If you access your mail via IMAP, this is easy (<rename-mailbox>, 
<create-mailbox> and so forth). If you access your mail locally, the 
easiest way for *creating* a mailbox is to put messages into it (i.e. 
if you put messages into a mailbox that doesn't exist, mutt will 
create it). That's the best way to rename mailboxes if you're not 
using IMAP: tag all the messages in a mailbox and save them to the new 
name. Deleting the old mailbox can either be trivial (if you're using 
mbox and you have $save_empty=no, then empty mbox files are deleted), 
or simply something that mutt doesn't handle (though I think I 
remember someone creating a patch that made $save_empty apply to other 
mailbox types as well).
