These all (including your 'flags may be wrong' bug) appear to have in common the arrival of unexpected FLAGS responses, usually for messages earlier than the current or last-fetched message. In my brief inspection I haven't seen any reason why your server should be sending these flag updates, but on the other hand it's perfectly legal thing for it to do. Thanks for the clue, I'll try to have a closer look at the code shortly.
