Sounds like terminfo issues - what does "echo $TERM" give you? You
stated you're using rxvt, try running "TERM=rxvt-color mutt". (Is
rxvt-color the right one? rxvt-xpm maybe? I cannot recall the exacts,
as I use rxvt-unicode)

Definately sounds like terminfo, as the TERM setting should be set via
your remote shell.
