So silly of me!  I had this line in my ~/.mutt/keybind (included from
~/.muttrc):
macro   compose \ed     "<pgp-menu>a0xBB9D2C24\n\n" "Sign with DSA key"

and was using that to sign.  This seems to prevent mutt from using the
remembered passphrase.

However, just pressing "p" then "s" made work fine!  Also, this works
fine too, doing the equivalent of "ps"

macro   compose I       "<pgp-menu>s" "sign"
