This seems to be an Exim problem. Is there something in its logs?

I set envelope_from in Mutt and this is not changed by Procmail. To
help addressing mailing lists, I have hooks like this one:
send-hook '~t mutt-users@xxxxxxxx' 'my_hdr From: mutt@xxxxxxxxxxxxxx'
