The problem with exiting immediately, attaching, and re-editing is that
you mess up send-hooks.  For example, my messages are spell-checked and
clearsigned with send-hooks.  To my knowledge, mutt doesn't have
"edit-hook"s, more's the pity :(

Speaking of send-hooks, an extremely hackish solution that should work
would be to prepend something like "[ATTACH]" to the "To" line, then
have a send-hook that deleted the "[ATTACH]" and then forced you to
attach something.  I'll play around with this and post some code (or an
explanation of why it won't work) later.
