Brendan's smtp patch allows this. (http://mutt.kublai.com/hg/mutt-smtp)
I think there are a few others too.
Plain mutt does not support smtp. You need an MTA (postfix, exim,
sendmail, etc.) or one of the small
relay only smtp agents. You can find an overview at

I recommend esmtp since I really love the identities feature.
