 $header_cache a directory)?

There is one header cache file per IMAP folder. (I am using GMail via
 IMAP).

 still not use the cache?

This was exactly what I did earlier to satisfy my own curiosity. The cache
 is still never used.

I am beginning to wonder if something on the system has become
misconfigured. This may be unrelated, but I was working on some code
earlier and noticed that "make" was rebuilding my whole source tree,
rather than just building object files whose source has changed. Am I
right in thinking this could indicate a system time based issue? Does mutt
use the system time in it's header routines?

I'm using ntpd, and after a "ntpd -s" the system time remains correct. I
have also verified that the filesystem time is recorded correctly, which
 it is.

I must apologise if this is not a mutt bug after all.
