I have this large patch to optionally use a gpgme based backend.
configure and runtime enable-able:

 ftp.gnupg.org/gcrypt/alpha/aegypten/mutt-patch-1.5.6cvs.g10.gpgme.6.gz

There is also an MDN patch.  However the approach on how to store the
seen message-ids has not thought out very well:

 ftp.gnupg.org/gcrypt/alpha/aegypten/mutt-patch-1.5.6cvs.g10.mdn.3.gz
