What's your $sendmail variable set to? You could try setting it to
'/var/qmail/bin/qmail-inject' (or wherever your qmail-inject binary is
hiding).
Unless there's something wonky with that setting, there should be no
difference between TO recipients and CC recipients. We can track it
down (e.g. create a shell script to make sure that the CC recipients
are being passed to the MTA), but if that's broken then there's
something very wrong with your mutt.
