Your question is worded incorrectly; mutt is a "complete" MUA. It isn't,
however, an MTA, so you need another program to take the mail that mutt
generates and send it elsewhere. That can be something like sendmail, or
ssmtp, or something else.
mutt does have basic built-in mail retrieval capability (POP and IMAP), but
many people prefer to use an external program like getmail or fetchmail to
retrieve mail from remote servers and stick it in the local mailstore for mutt
to read.
