The Japanese edition of mutt works pretty well out of the box:

OS (NetBSD) environment settings are:
LC_CTYPE=ja_JP.eucJP
LANG=ja

Still have problems synchronizing the default editor's (nvi-m17n) automatic
encoding recognition and mutt's encoding conversion with iconv.  Simply
zero time to study and correct the situation, so I use procmail to forcibly
filter all Japanese mail through nkf or iconv to change it all to euc-jp.

I have not gotten utf-8 to work, but it is more an OS issue than a mutt one.
No time to work on this either; sorry.
