Leaking bcc headers ???



Recently, somebody sent me this:

Obviously, mutt is my MUA.  Less obviously, I am on Debian and using
exim:

    Exim version 3.36 #1 built 11-Dec-2003 11:35:23


I have noticed my bcc headers in my messages on some recipients'
systems, and I never followed up.

What is causing this?

What can be done about this?

What do you think?
