The undergoing effort for having an extensible Mutt (aka. mutt-guile [0])
aims at making it easy to perform this sort of mail processing, among
other things -- although I'm not 100% sure you'd be able to do what you
want with it in its current status.

Alternatively, Guimb and Sieve [1] are specifically designed for mailbox
processing.
