I can only guess here. There are two most likely reasons:
- you are delivering directly from a dial-up host
  Most MTA reject mail from dial-up lines as a spam protective
  measurement. Use a smart/relay host.
- The envelope address points to a non-resolvable host.
  Check $use_from and $(use_)envelope_from they should both be set.
