Nope, you can use multi-letter sequences. The only caution I would
have there is that mutt uses timeouts to disambiguate between things
that overlap. For example, if you use 'c' for changing mailboxes and
then assign 'cI' to a macro, then when you press 'c' you have to wait
a second or two before mutt will realize that you meant to change
mailboxes and not trigger a macro. (Fairly obvious if you think about
it.)

Nope, you can assign anything you like to whatever you like (that's
why, in macros, it's better to use a direct <function> call (e.g.
'<change-mailboxes>') rather than a keypress (e.g. 'c').
If you wanted to, you could totally reassign all of mutt's keys so
that it did things like behaved more like pine or vim or emacs.
