Hmm, your $sendmail program is exiting badly. What is $sendmail set
to?

That's how mutt behaves if and when the $sendmail program returns a
failure exit code (as it has in your case).

Mutt saves the mail into the "sent" mailbox before attempting to send
the message (that way you can't have a message sent that has not been
recorded). So, if sending the message fails or appears to fail, you
will always end up with a copy of the message in your sent mailbox.
