Are you sure your imap server supports either digest-md5 or cram-md5
authentication? It may not. If you take both of those lines out of
your muttrc, what happens?

imap server supports cram-md5 at least.
I build mutt-1.5.12 as follows,
./configure --enable-pop --enable-imap --with-ssl=/usr --enable-debug
make
Now, it works!
