You can't use "my_hdr" with a To: header to modify the reply address
with the current Mutt version.
You can do some fixup on the realname only with the reverse_reply patch: [1]

On a side-note, can't you just tell that person that his Reply-To:
header sucks and get him to use a proper one? People tend to have a
reason for using that header.
