After some testing, it appear that the Exchange server does return all
FETCH responses with something like "FETCH 1:n (FLAGS)" (this seems to be
what Thunderbird does at startup). Another user verified that fetching
~2000 messages at a time seemed to not trigger missing FETCH responses, so
we probably need to fetch the headers in blocks.
