A message in mbox format is considered read when it has the header:

    Status: RO

The MTA does not strip this header, so it is possible to send somebody
a message which appears to already have been read, if they store their
mail in mbox format.  Normally, of course, MUAs (including mutt) don't
do this.

You could try looking for this header in the mail spool when you
aren't running mutt, to see if that's the problem.
