In addition to the s->S thing, there are also notifications in the
status line, and cleaner in-band signalling in more recent versions:
You'll notice that the attachment markers ([-- ... --]) in classical
signature fake mails are colored as plain text in recent mutt
versions; they would be colored as attachment markers with older
versions of mutt.

The code behind the cleaner in-band signalling is some kind of
"cookie" that's generated once per mutt session, and unique for each
session.
