mutt doesn't correctly display attach name and file name in case attachment has
MIME-encoded header such as shown below (captured from the raw message):

Content-Type: application/vnd.ms-excel; name="=?KOI8-R?B?8NLJzF8xX0RNUy54bHM=?="
Content-Disposition: attachment; filename="=?KOI8-R?B?8NLJzF8xX0RNUy54bHM=?="


In a pager I expect to see decoded attachment name (in the same way as I see 
decoded subject line)
but I see the following text instead:

Use of RFC2047 encoding in MIME parameters is explicitly forbidden
by the relevant standard.  Mutt has a parameter,
$rfc2047_parameters, that controls whether mutt decodes this kind of
parameter.
